Important Patient Information

• Payment & Fees: We accept cash, debit or Pay by Bank. ATMs on-site and a $3 state fee required for all orders.
• Documentation: A valid ID and your Utah Medical Card must be presented for all purchases. First-time visitors must visit the pharmacy in person to complete their profile setup.
• Order Fulfillment: Online orders are processed for same-day pickup only.
• Promotions & Coupons: Sale prices apply only to same-day pickups. All coupons must be verified in-store (no third-party codes).
• Return Policy: Returns are for defective items only and accepted within 7 days with the original receipt and packaging.
• Consultations: New medical cardholders are required to schedule a pharmacist consultation before their first purchase.
• Medical Card Services: Bloc does not issue or renew medical cards. You can find a local provider to assist with the card process.
